Services We Offer:

 

Offering a wide range of patient services:

  • Medications related to the hospice diagnosis

  • Medical supplies and equipment required to ensure patient comfort

  • Assistance with personal care and activities

  • Support for the patient and family with emotional and spiritual concerns

  • Coordination of community resources

  • Education for family and caregivers

  • Volunteers to provide companionship

  • Special services for speech, diet, physical and occupational therapies

  • Bereavement services for family and friends

Hospice Criteria:

 

Eligibility requirements for Hospice will be determined when the patient meets the following criteria: 

  • The patient has a chronic illness such as:​​​

  • Alzheimer's​

  • ALS

  • Cancer

  • Cardiac Disease

  • Debility

  • Dementia

  • HIV / AIDS

  • Liver Disease

  • Pulmonary Disease

  • Renal Disease

  • Stroke

  • and others

  • Terminal diagnosis with a limited life expectancy of 6 months or less if the conditions take its natural course

  • The patient's physician and hospice physician authorize hospice care

  • The focus will be on palliative treatment, including pain control and emotional or spiritual support.
